The Cremation Authorization form is required for us to complete a cremation. We cannot complete a cremation without this form filled out entirely. If you have any questions or would like clarification on anything within the form, please don’t hesitate to give us a call or send us an email. Please note that after submitting these forms to us, we will never cremate your pet until we have received verbal confirmation from you to do so.
